How much does health insurance cost?

5 min read · Reviewed by licensed agents

The sticker price and what you actually pay are two very different numbers — here's how health insurance pricing really works.

Health insurance pricing confuses almost everyone, because the 'sticker' premium and what you actually pay after subsidies can be wildly different. Here's how to think about the real cost.

What drives your premium

  • Your age (older = higher premiums).
  • Where you live.
  • Tobacco use.
  • The plan's metal tier (Bronze, Silver, Gold, Platinum) and network.
  • How many people are on the plan.

Subsidies change everything

On the ACA marketplace, premium tax credits lower your monthly cost based on income and household size. Many enrollees qualify for substantial help and pay far less than the listed premium — which is why comparing your real, after-subsidy price matters more than the sticker number.

Premium isn't the whole cost

A plan's true cost includes its deductible, copays, and out-of-pocket maximum. A low-premium Bronze plan can cost more overall if you use a lot of care; a higher-premium Silver/Gold plan may be cheaper for someone with regular medical needs. Compare total expected cost, not just the premium.

How to find your real number

Estimate your annual income (for subsidies), list your expected care and prescriptions, and compare plans on total cost. A licensed agent can run your actual after-subsidy pricing at no cost.

Frequently asked questions

It depends heavily on your age, location, plan tier and — crucially — the subsidies you qualify for. Many marketplace enrollees pay far less than the sticker premium after the premium tax credit. The only accurate number is your personalized after-subsidy quote.

Listed premiums are before subsidies. Once income-based premium tax credits are applied, many people pay a fraction of that amount. Always check your after-subsidy price.

Not always. A low premium with a high deductible can cost more overall if you use a lot of care. Compare the total of premium + deductible + out-of-pocket max for your expected usage.
